The Team at Oak Street Health, specializing in personalized primary care for older adults, Licensed Clinical Social Worker (L.C.S.W) will lead this seminar to discuss targeted ways and best practices to reduce stress to improve your health, lifestyle and provide positive stimulus.
Additionally, Oak Street Health (OSH) Outreach Executives will highlight the ways OSH Primary Care Centers specifically focus on the older adults population including insights on enhanced services available to improve overall physical wellness.
